To ensure the safety and health of all dogs in our care, current vaccination records from a licensed veterinarian are required prior to participation in any training or Board & Train program.

Please upload your dog’s most recent vaccination records below. Records must clearly show:

• Dog’s name

• Owner’s name

• Veterinary clinic information

• Dates of administration and expiration

Required Vaccinations:

• Rabies

• Distemper/Parvo (DHPP/DAPP)

• Bordetella

Strongly Recommended Vaccinations:

• Leptospirosis

• Canine Influenza (CIV H3N2/H3N8)

We strongly recommend all vaccinations be administered and documented by a licensed veterinarian.
